Thank you for agreeing to serve as a faculty mentor for a student team that is agreeing to work under your close supervision to design and facilitate an anti-racism/anti-Black racism course in Winter Quarter 2024.

Student teams are required to submit a separate application by June 7, 2023 at or before 11:59PM PST. However, their file is not complete and their candidacy will not be considered unless their faculty mentor has SEPARATELY submitted this form.

By submitting this form, I acknowledge that I have read the agreement below and agree to supervise the student team to develop a 1-unit course for which I will approve the syllabus that will be distributed to students, visit at least one class, assign and submit grades, and receive the evaluations of the curated materials at the end of the quarter. I understand that I will receive $1,500 in a research/creative products account. I have agreed to serve as the faculty of record for a course being facilitated by the following students:

2. As a faculty mentor, I agree to:

affirm that the students named above have the intellect and character to be effective and responsible facilitators/curators of an academically useful course which can be adequately prepared by the end of Winter quarter 2022.

affirm that I have discussed the proposed course concept with the students named above and have approved the proposed course plan for submission as part of the students' application.

meet regularly with the students during Fall Quarter 2023 to guide their development of the seminar syllabus and will approve the version of the syllabus to be distributed to the students as I will serve as the faculty of record; and

visit the Winter Quarter 2024 course at least once, be available as a continuing resource for the students, and serve as the instructor of record for the course (including submitting final grades) during Winter Quarter 2024. I understand that I am not obligated to visit every course meeting.

Please write a brief letter of support for the student applicants you have agreed to mentor with your sense of the contribution that the course can make to anti-racism efforts in your field and why the students working with you are a good fit for this task. What would you expect the outcome to be for the 25 students in the class? (300 words max).
